Transitional care refers to the 'transition' period between a hospital stay and returning home. After a stay in hospital you may be advised or assessed to receive transitional care to help get you back on your feet and make the move back in to your home easier. Transitional care is offered on a short-term basis for up to 12 weeks, and can be provided either in residential care, at home or a combination of both depending. on your health and individual needs.
At Alwyndor, we provide ten dedicated rooms for Transitional Care and an at-home Transitional Care Program (TCP) run by our Home Support team. Our program provides goal orientated and therapy focused care to ensure you regain your confidence and independence to get back in to your own home as quickly as possible. Depending on the level of care you require, our services may include access to a social worker, nursing support for things like wound care, personal care and podiatry or low-intensity therapy to improve your mobility, strength, balance, and speech.
To be eligible for a TCP, (while you're in hospital) you will need to be assessed and approved by the Aged Care Assessment Team (ACAT). They will discuss transitional care suitablility and explain your options to you. It's all about what's right for you for your transition back to home.
